New Client Therapy Request Information 

New cash clients please read and complete the form below -

New clients must complete the form below and submit their information, Once the form is submitted you can schedule an appointment by clicking here - Appointment. You will receive an email from me with invitation and  instructions on how to create your account and password at the telehealth portal. If you do not receive an email within 24 hours of your request, send an email to ldery2016@yahoo.com or you can text me at (734) 968-0218.  Check your spam folder for the telehealth portal invitation. Once your appointment is paid and scheduled you will receive text message reminders. 

 New clients using their Michigan health insurance please follow these instructions-

 You will be directed to a webpage to complete your information and submit your health insurance information. Once your insurance is verified you will be able to schedule an appointment on my calendar. Be sure you are scheduling with Louise Dery, LCSW. Once  your 30 minute initial appointment is scheduled, you will receive text and email reminders. All follow up  sessions are 55 minutes. click this link at Brightside Health to get started.

Cancellation policy & Scheduling

Cancellation

Cash clients- are required to text or email to cancel their scheduled appointment within 24 hours of the appointment day and time. This will prevent a late cancel fee, which is the same amount as the session fee. 

No Show- Clients who no show for a scheduled session will also pay the fee equal to the session fee. .

• Clients Pay In Advance -

Clients pay in advance to secure their preferred appointment day and time. They may cancel and use the payment for another appointment, but it must be within 2 weeks of the cancellation This does not apply to no show. No show clients will forfeit their payment due to a missed appointment.  

Appointments-

Counseling is not offered by subscription. Each session is paid in advance of your therapy session to secure your preferred day and time.

Stripe, a payment processing platform,  accepts credit & debit MasterCard, Visa, American Express, Discover, HSA and FSA insurance cards.

Michigan Insurance Clients-

Clients using their insurance will comply with the policies at Brightside Health- Michigan regarding no show and missed appointments.
